Cause Analysis Training 

This two-day training will educate key quality and risk individuals on HPI methodology when performing Cause Analysis for serious safety, reportable, and sentinel events. The course will accomplish the following:

  1. Outline Cause Analysis fundamentals
  2. Review organizational structure and the new role of leaders
  3. Apply steps of the Root Cause Analysis (RCA) process
  4. Include simulation practice
  5. Aid in development of corrective action plans
  6. Assist Intermountain in developing a stronger RCA program

This training is geared towards: Quality, Risk Management Simulation leads, and key departmental managers. 

Upon completion of this training, participants should be able to:

  • Define new team roles and their application in the Cause Analysis process
  • Describe the RCA three meeting model
  • Demonstrate effective interview methods
  • Observe simulation then incorporate observations into small group work
  • ​Complete the new form- Cause Analysis Report
The following are the four two-day training options: 

  1. May 11, 2016 – May 12, 2016 at Ogden Courtyard Marriott
  2. June 29, 2016 - June 30, 2016 at Intermountain Homecare & Hospice
  3. October 19, 2016 - October 20, 2016 at LDS Hospital
