Provider Benefit Tools / CareAffiliate

Access to secure member information via the Provider Benefit Tool / CareAffiliate is only available to providers and facilities contracted with SelectHealth.

Noncontracted providers can call Member Services at 800-538-5038 for benefits, eligibility, and claims information.

To set up access to your patient's claims and eligibility information online, submit the Login Application and Information Technology Services Agreement (ITSA). Both documents are required if you are requesting access for the first time. To add a user to an existing account, only the log in application is required.

SelectHealth Information Technology Services Agreement​ (ITSA) –​ This is an agreement between your office and SelectHealth regarding access to the SelectHealth system. Only pages one and 12 are required to be completed and returned.


Online Login Application – (There may be some functional limitations if using Internet Explorer) This is the official request for access. All new users need to be listed on this form.
