​​​​​​Care Management Resources 
As part of SelectHealth member benefits, we maintain a trained care management team of licensed mental health providers and registered nurses who work closely with providers and patients to help better manage chronic or complex conditions, coordinate complex care, and ensure follow-up support. For SelectHealth members, there is no extra cost for care management services.


What do care managers do?​

Care managers work with our Medical Home team to ensure members get the proper treatment and medications. They create a care plan to improve patients’ overall health and well-being. They also help members:

  • Get preventive care
  • Manage chronic conditions
  • Understand their benefits
  • Connect with community resources for additional help


SelectHealth care managers help guide patients assigned to the Restriction Program (as mandated by Utah Medicaid).

Who should a provider refer for care management services?

Trained care managers support your practice by coordinating care for:

  • Patients needing complex care. Patients who have multiple chronic conditions and need care coordination, such as navigating services and benefits as well as complex care management. We can help them set goals to maintain good health and coordinate with their care team to ensure health goals are being met
  • Those impacted by social determinants of health. We help patients who experience transportation issues, food insecurity, poor health literacy, underlying mental health conditions, or other similar needs to connect with appropriate community resources.
  • Patients needing chronic disease management. We can help patients set goals and manage cancer, diabetes, organ transplant, chronic obstructive pulmonary disease (COPD), asthma, and other chronic conditions.
  • Pregnant patients. We offer our Healthy Beginnings program for maternity care.
  • Children and adults with special needs. We can help these members navigate care and benefits.
  • Patients included in the Restriction Program as mandated by Utah Medicaid.  We help care manage patients who overutilize providers, prescribers, pharmacies, and the ER.


How can you help SelectHealth members learn more about Care Management?

We provide a basic information rack card​ for use in your waiting areas. We invite you to review a copy of this card online. If you would like a supply of these cards mailed to your clinic, contact your Provider Relations representative at 800-538-5038.


Members can talk with a care manager by calling 801-442-5305 (or toll free at 800-442-5305), weekdays from 8:00 a.m. to 5:00 p.m.​
