Home Visits for Readmission Avoidance

Effective January 1, 2014, SelectHealth implemented a new Care Management readmission avoidance program for SelectHealth Advantage and SelectHealth Community Care members. This program may include a single home visit from Intermountain Homecare on behalf of SelectHealth to members enrolled on your home health service. This one-time visit is for selected members at high risk for readmission. The visiting nurse will assess the member’s understanding of his or her disease, the member’s ability to complete follow-up instructions, the availability of caregiver support, and several other measures that can help avoid a readmission. The visit is separate from the member’s home health benefit, and it is not intended to replace or be a substitute for the home health services you provide. We will coordinate care with you to help prevent future admissions.

If a member on your service receives this home visit, please be assured that it will not affect the member’s home health benefit limit or the coverage of home health services.
